Patient Transfer and Access Center

Through the Patient Transfer and Access Center, providers can now request a transfer from their acute care hospital to Mass General Brigham hospitals—all with a single phone call.

Ensuring easy access to the right care, at the right time, in the right place

With a single phone call, the Mass General Brigham Patient Transfer and Access Center (PTAC) works with providers to facilitate efficient acute care hospital transfers to either Brigham and Women’s Hospital, Massachusetts General Hospital, or one of our acute care community hospitals. An integrated and centralized hub, the PTAC is staffed all day, every day by a knowledgeable, experienced, and coordinated team of access, bed, and case management specialists who will provide a seamless patient transfer.

Our team will assess the patient’s health, condition, and needs and use real-time, systemwide capacity data to match them to the most appropriate level and location for inpatient care. On a case-by-case basis, our team may explore the option to transfer the patient back to their original hospital for recovery, once care is complete.

How to transfer a patient

For providers looking to transfer a patient, please call us at 1-855-468-6642 (1-855 GO TO MGB).

Referrals and Transfers

  • Loyola Medicine Referral App

We're Committed to Strong Partnerships and Better Outcomes

When a patient is referred or transferred to Loyola Medicine, we view collaboration as a truly invaluable asset. We believe that through cooperation, we can find the most effective way to care for patients—care that goes beyond the medical condition to treat the whole person.

We are committed to a smooth transfer process and know how crucial clear, efficient communication is. With that in mind, we have a dedicated physician phone line for patient transfers:

Loyola Physician Transfer Line: 855-4-LOYOLA / 855-456-9652

We also have a dedicated phone line, for physicians only , to facilitate an appointment with a specialist:

Loyola Physician Referral Line: 708-327-DOCS / 708-327-3627

Our concierge service helps referring physicians navigate Loyola’s network of providers. Referral nurses are available Monday-Friday, 8 am – 5 pm, to facilitate and triage the appointment to the appropriate Loyola provider. Our nurses will work with the referring physician offices and the Loyola providers to ensure that the patient is seen within the timeframe requested or sooner, based on acuity. After hours, you may leave a message for a return call the next business day.

To streamline referrals, Loyola offers a free mobile app, Referral , which connects referring physicians with Loyola specialists.

When you refer a patient to Loyola, our commitment is to keep you informed of our diagnosis and treatment plan, as well as to refer the patient back to you for follow-up care.

Physicians who refer patients to Loyola can directly access their patients' electronic health records through LoyolaConnect, our secure physician site.

Regional Patient Transfer Service

Patient Transfer Service was developed to better serve the needs of patients within our region, to improve access, and to facilitate patient transfers and direct admissions from area hospitals and community physicians.

Baystate Medical Center

Toll free number 1-877-790-2345 is available for direct admission and transfer requests.

HOW PATIENT TRANSFER WORKS

Experienced Patient Placement Managers (RNs) will manage your patient transfers and direct admission calls, and coordinate care between referring facility and the accepting physician.

When a transfer call or direct admission request is received, the Patient Placement Manager will page the appropriate physician identified to accept the transfer. Within fifteen minutes, the Patient Placement Manager will coordinate a three-way conference call between the requesting MD and accepting MD. Once the physician has accepted the transfer, the Patient Placement Manager will verify the level of care, locate the appropriate bed, facilitate coordination of patient transportation, initiate follow up communication with the referring organization or physician, and facilitate patient transfer to a Baystate Health hospital when the bed is available.

We look forward to working with you, and to receiving your feedback as we move forward with the Patient Transfer Service. Please don't hesitate to contact Alicia Meacham, Program Director at 413-794-1709 with questions about this service.

Baystate Children's Hospital

To directly admit or transfer a pediatric patient to Baystate Children's Hospital, call 4-KIDS (413-794-5437)

How Patient Transfer Works

  • Patient placement will set up a three way call with the resident holding the admission pager (the “Grid master”). The Grid master may need to first discuss the patient with the charge nurse and/or hospitalist on-call to determine whether there is a bed/staff available, and whether this is the appropriate level of care.
  • The Grid master lets the charge nurse know about the admission and the patient is added to the bed board for patient tracking on the unit.
  • Bed placement should be instructed to call the referring physician and family for the patient to come in after the request has been placed and an encounter made. If there will be a delay in getting a bed, the intermediary disposition (i.e., waiting in the office vs going home first) should be discussed with the referring physician. If the family arrives prior to a bed being available, or if they do not have a care encounter, the process will be delayed.

The Grid master is your main contact on the day of admission. After the day of admission, please call 413-794-2398 to be connected with the attending hospitalist caring for an admitted patient.

Conditions not appropriate for direct admission

  • Being in respiratory distress / increased work of breathing
  • Recent seizures, loss of consciousness, or change in mental status
  • Concerns for cardiac problems
  • Concerns over sepsis
  • Immediate need for antibiotics, intravenous fluids, oxygen or respiratory treatments
  • Bleeding from any site
  • Acute traumatic injury

For non-elective admissions, patients will need to have been seen by a health care provider on the day of admission.

Find a UVA Health Provider

At UVA Health System, we are continually evaluating our procedures to ensure we are providing optimal access to care. Your satisfaction with the patient transfer process has been a key consideration this year, and after careful review, we have made improvements we hope will allow for more streamlined service and a reliable, timely response to transfer requests.

For those requesting to transfer patients to UVA, the process will appear unchanged, with the exception of the phone number required. Behind the scenes, however, we have established our own internal response team within our Transfer Center. These 12 new personnel are medics — not call center operators — who have up to 30 years of experience in the medical field. This means they are better prepared to appropriately triage patients based on information provided. They can more easily detect patients who are emergent and they know which clinicians to contact for consultation.

“We now have Medic Transfer Coordinators working the transfer service 24-7,” says Transfer Center Manager Sarah Souter. “By asking key clinical questions, they can better guide calls to the appropriate service line, and can be a better monitor of patient conditions if changes occur.”

In addition, a new note section in Epic will allow UVA to record and track this information over time so that when a patient arrives, UVA’s team is well aware of their current status. “Now we have real-time clinical information that allows us to place patients appropriately from the start,” Souter says.

Because the transfer team will be co-located with the UVA Bed Center, there will also be greater transparency in terms of bed availability. “This will allow us to better estimate when exactly a bed will be available and provide a clear ETA to referring hospitals,” says Souter.

If you would like to transfer a patient to UVA Medical Center, call 844.XFERUVA (844.933.7882).

Inter County Transfers

An Inter County Transfer (ICT) is the transfer of Medi-Cal (MC) benefits from one county to another to ensure no interruption or overlap of MC benefits when a client permanently moves from one county to another within California. The client must not be required to complete a new application when moving active MC benefits between counties in California. MC cases must remain active throughout the ICT period with no interruption in benefits. The Receiving County must make every effort to contact the Sending County, and not the client, for needed information/verification.

For the purpose of ICT counties are described as either the sending or receiving county, as follows:

  • The county where the client moved from (former county of residence)
  • The county where the client moved to (new county of residence)

Important: If a client reports an Out of State address, the EW must discontinue the case and send an adequate Notice of Action, no ICT is required. If a client is temporarily residing in another county or State, an eICT must NOT be initiated.
